In the next activity, you will create a survey using a Google Form to be shared with other students in this class. 

First, you have to decide on some interesting Multiple Choice questions that you can include in your survey. Questions with more than one possible answer are Multiple Choice questions. Your survey should have no less than 4 questions. Some examples are listed below.

Sample Questions (think of your own!):

How old are you? Answers options 11, 12, 13, 14

Are you male or female? Answer options Male, Female

How many siblings do you have? Answer options 1, 2, 3, 0, more than 3

Once you have come up with four questions, follow the instructions on the next page to create a form.
